Position Title
Disability Support Worker
Role Purpose
To provide respectful and person-centred support to a 55-year-old participant with mental health needs, assisting with daily living activities, community participation, and overall wellbeing while promoting independence and choice.
Key Duties
Support the participant with daily living activities such as meal preparation and light household tasks
Provide emotional support and encouragement in a calm and respectful manner
Assist with community access, appointments, shopping, and social activities
Support the participant to build confidence and independence
Follow care plans, risk assessments, and support strategies
Monitor wellbeing and report any concerns to the coordinator or manager
Complete progress notes and timesheets accurately
Skills & Qualities
Patient, respectful, and reliable
Good communication skills
Understanding of mental health support
Ability to work independently and follow instructions
