Be Part of Something Bigger: The Role

As a Disability Support Worker, you will be instrumental in providing high-quality, person-centred care to our clients across various settings, from their homes to community participation activities. Working closely with the Client Liaison Officer, you will assist clients with their daily living needs, support community participation, and contribute toward their personal and NDIS goals. Your role is vital in fostering autonomy and a fulfilling lifestyle for those we support.

 

Your Key Responsibilities:

• Build Strong Relationships: Be the front facing contact for clients and families, fostering trust and positive interactions.
• Deliver Quality Services: Assist in day-to-day tasks, ensuring clients achieve their personal and NDIS plan goals.
• Support Compliance & Documentation: Complete accurate documentation and progress notes to align with Breakaway standards, adhering to policies and procedures of the organisation.
• Engage in Community Participation: Provide clients with opportunities to engage in social and community activities and support them to actively participate.
• Manage Risk and Behaviour Support: Follow positive behaviour support plans and manage risks as required.

 

What You Bring to the Table:

• Compassion and Resilience: A caring attitude with the ability to build rapport with clients, families, and teams.
• Disability Support Experience: Prior experience in disability or community services is a must, and mental health experience is highly regarded.
• Strong Communication & Organisational Skills: Ability to collaborate effectively with various stakeholders and manage time effectively.

 

Qualifications & Experience:

• Essential: Current Blue Card, NDIS Worker Screening Clearance, and Driver’s Licence.
• Preferred: Certificate III or IV in Disability or Mental Health or related field; demonstrated experience in mental health or complex behaviour support is highly regarded.
• Skills: Proficiency in basic administrative tasks and intermediate computer skills.
