• Date & Time: Starting Thursday, 8th May 2025, every 2 weeks on Thursdays OR Fridays, 11:00 AM – 2:00 PM
• Duration: 3 hours
• Location: Miller Street, Yea, VIC
• Service: Respite Care (In-Home Respite & Accompanied Outing & Social Support)
• Notes: Must have a female worker. Client prefers in-home services. Flexible booking on Thursdays or Fridays between 11:00 AM – 5:00 PM. Tasks include arranging meal delivery with Valley Meal Prep, accompanying to the chemist and library, and assisting with domestic tasks if time allows (tidying kitchen, changing bed linens, or laundry). Vehicle must accommodate Lyn's 4WW (Large wheelchair).
• Mobility: Lyn walks unaided indoors, uses a 4WW outdoors, and requires supervision due to a high fall risk. Hold her hand for support when getting in and out of the car. Pick-up from Lyn's home.
