- Client: G. Hill
- Address: McDonalds Road, Epping VIC
- Service Type: Social Supports
- .Shift Type: Ongoing – Every 2 Weeks (Fortnightly)
- Service Schedule Day: Thursday (fortnightly)
- Time: 9:00 AM – 11:00 AM
- Client Flexibility: Weekdays anytime between 9:00 AM – 5:00 PM Service Details Participation in social and community activities Shopping assistance (local grocer) Assist client to visit the bank beforehand
- Client Considerations Medical: Type 2 Diabetes
- Mobility: Independent
- Continence: Catheter in use (changed every 6 weeks)
- Communication: English
- KM Approved: 20 km Behaviour
