Job Responsibilities:

• Providing in-home care and support to clients, including assistance with personal care, meal preparation, and household tasks
• Assisting clients with mobility and transportation to appointments and outings
• Building rapport and developing relationships with clients to ensure their comfort and well-being
• Communicating regularly with clients and their families to provide updates and address any concerns
• Maintaining accurate records of care provided and reporting any changes in client's condition to the supervising nurse or case manager

Job Requirements:

• Must have a full Australian driver's license and access to a reliable vehicle for transportation
• Valid police check and working with children check
• Previous experience in home and community care or a related field is preferred
• A compassionate and caring attitude towards clients and a commitment to providing high-quality care
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Flexibility to work a variety of shifts, including evenings and weekends
