• Provide person-centered support with daily living activities including personal hygiene, grooming, dressing, toileting, and mobility assistance.
• Assist with household tasks such as light meal preparation, tidying, laundry, and shopping support when required.
• Support medication prompts and follow care plans under supervision of accredited clinicians.
• Accompany clients to appointments, social outings, and community activities to promote engagement and wellbeing.
• Observe and report changes in client condition, behaviour, or environment using accurate documentation.
• Uphold client dignity, privacy, and cultural preferences while maintaining safety and infection control standards.

• Has Car: Yes — ability to travel to client homes across Hornsby and surrounding areas.
• License: Full Australian Drivers Licence with a clean driving history.
• Police Check: Current national police background check.
• Working with Children Check: Valid working with children clearance.
• Experience in personal care, disability support, aged care, or similar roles preferred.
• Strong communication, reliability, empathy, and basic literacy for record keeping.
