Description:
- Showering Level of assistance: Client prefers to independently shower herself but as she is high falls risk, she needs supervision. At times she mightneeds assistance.
- Equipment in use: shower stool, grab rails, non-slip mats
- Mobility /Transfers: independent with a 4-wheel walker
- Setting up area (clothes, towel etc): in bathroom
- Undressing/ dressing: Client will try independently but might need support to don and doff clothing depending on health and severeness of shortness of breath on the day
- Assistance in the shower: Client will try independently but might need support
- Getting dry: Client will try independently but might need support
- Special requirements: towels, toiletries in bathroom, prompt client to bring in clean clothing - please report skin break, wounds, bruises or other concerns to Care Manager
- After shower: walk client to lounge and then clean/tidy up the bathroom Accompanied outings/respite
- Behaviour considerations/ alerts: Shortness of breath, oxygen 24/7
- Activity ideas/ locations: If time permits after the shower client might ask for some shopping errands at Woolworth Churchill
- Communication: Good
- Equipment/ Mobility: 4 wheel walker Toileting/ continence support? self
- Km's approved: 5 km Special
- Instructions: Client might ask for support with food shopping or task at home if time permits after the shower.
- Access to property: Front door
- Parking: Street parking
- Alerts: Health issues with severe shortness of breath, 24/7 oxygen concentrator or cylinder
- Family present during service delivery: daughter or son in law
