Description:Personal Care / Support SW available for client Chiu-Lin Tsang from 10:15 to 12:45 Medical transport/PC as one off on Monday 15/12 Destination: Balmain Hospital Strong Clinic, 29 Booth Street, Balmain, NSW 2041 Purpose: Group physiotherapy class (11:00 am – 12:00 pm) Schedule:Monday, 15 Dec – 10:15 am to 12:45 pm (2.5 hours) Support Worker must be female: Pick up the client from Glen Osmond House and transport to the clinic. Stay at the clinic during the session to assist with needs such as bathroom support. Transport the client back to Glen Osmond House after the session.
