Description: Access to property: walk up driveway to front door and knock (doorbell not working currently) Parking: on left side of home (near basketball rings) Alerts: - client has had a previous stroke and so walks slowly, please do not rush him as he is walking with his 4 wheel walker - client can become dizzy if changing positions too quickly - always check for dizziness, and change positions slowly - advise Pina if client experiencing dizziness that will not settle, such that she can check his blood pressure - client has limited standing tolerance (<5mins) due to lower limb pain and numbness - please complete personal care tasks in seated position for safety - Property located near bushland, please be on the lookout for snakes (note none recently seen) Family present during service delivery: (who lives with the client or visits regularly): Giuseppina "Pina" (wife), Luca (Son) SHOWERING Level of assistance: offer assistance at each stage of showering (as amount of assistance required may vary) Equipment in use: shower chair Mobility /Transfers: supervision with 4 wheel walker during support worker shift (family otherwise supervise at other times) Setting up area (clothes, towel etc): Pina (wife) will put out fresh clothes and continence pull-up pants. Towels present in bathroom Undressing/ dressing: offer assistance for each item of clothing Assistance in the shower: offer assistance for each task (note; client likely wishes to wash private parts / genitals himself, but may need assistance with washing his back, legs, hair etc) Getting dry: offer assistance to dry each part of body (same comments as washing) Special requirements: - after showering, offer assistance for grooming tasks - shaving face, brushing hair - Always make sure to monitor for any dizziness, and change positions slowly, + advise Pina if client experiencing dizziness such that she can check his blood pressure After shower: walk to loungeroom to sit in electric recliner chair (or sit on dining chair in between if too much to walk there all in one go) OTHER TASKS - Please check with Pina (wife) - in most instances, it would be appreciated to strip the bed sheets, then wash in the washing machine -> hang on outdoors line (or dryer if bad weather) - Please note bed sheets / kylie mat may be soiled with body fluids - please wear appropriate PPE as necessary - Check with Pina for other light domestic assistance tasks that may be helpful if time allows (she can provide details of tasks, locations of cleaning equipment etc)
