Description: MALE SUPPORT WORKER ONLY Personal care 1 hour Showering Level of assistance: Supervision- leaning towards physical assistance Equipment in use: Shower chair Mobility /Transfers: Walter uses his four-wheeled walker and walking stick Setting up area (clothes, towel etc): towel is on the rail in the bathroom. Undressing/ dressing: Only supervising, help when he needs. Assistance in the shower: Assisting Walter only if he needed Getting dry: No assisting just supervision. Special requirements: Please ask discuss with family the level of assisting. The client's daughter suggested that client is able to do it independently however due to his fragile and falls it is best to assist him as much as we can. After shower: Please clean the bathroom Domestic Assistance 1hour: aim for twice a week. -Location of cleaning products: Supplier to bring their own. -Additional tasks: None -Special instructions/ Preferences: Bring your own cleaning supplies, mops and vacuum. -Client participation: None -Tasks: • Change the linen and put new linen in bed • Hang washing out if available • Clean toilet (including toilet bowl) • Clean bathroom (including vanity, mirrors, surfaces and shower glass) • Clean Kitchen (bench, microwave) • Sweep and mop all floors Access to property: Access through the front door, ring the bell (this will be updated on 29/12) Parking: There is free parking along the street Alerts: Client is unsteady on their feet. Client has fall history/fragile Please discuss with family re: personal care Family present during service delivery: Client lives with wife (Irma), and daughter (Christi) will be presented during the service
