Description: ONLY 1 hour or 1 1/2 hour Female Showering Please note: Before a shower- please consult and discuss with client's preferences and how much assisting she requires. Sandy has weakness on one side of her body from a stroke. Level of assistance: Physical assistance Equipment in use: Shower chair Mobility /Transfers: Sandy uses his four-wheeled walker and walking stick Setting up area (clothes, towel etc): towel is on the rail in the bathroom. Undressing/ dressing: Requires assisting with undressing and dressing Assistance in the shower: Assisting Sandy to wash her back only if she needed- please ask Sandy Getting dry: Please assist with drying her particularly her back area- she is unable to reach Special requirements: Please ask to discuss with family the level of assisting. After shower: Please clean the bathroom afterward. Cleaning 1 hour after PC Domestic Assistance 1hour: -Location of cleaning products: Supplier is located in the laundry room- ask client. -Additional tasks: None -Special instructions/ Preferences: -Client participation: None Tasks: • Change the linen and put new linen in bed • Hang washing out if available • Clean toilet (including toilet bowl) • Clean bathroom (including vanity, mirrors, surfaces and shower glass) • Clean Kitchen (bench, microwave) • Sweep and mop all floors Access to property: The house located in Avington community. To access the unit 809 drive up and turn left on to Kentya Street. Access through the front door, ring the bell Parking: There is a park at her driveway Alerts: Client is unsteady on their feet. Client has fall history/fragile/weakness on one side of body Please discuss with family re: personal care Family present during service delivery: Client lives with husband (Dave)
