💰 Salary: $85,000 – $110,000 + super
✨ Salary Packaging: Up to $15,900 tax-free + additional $2,650 per year (meals & entertainment)

About the Role

We are seeking an experienced Registered Nurse to join our Community Services Team, supporting older people to live safely and independently at home under the Support at Home Program.

This role combines clinical care, care coordination, education, and advocacy, working across home care services, group social support programs, and seniors’ activities. You will deliver person-centred care with a strong focus on wellness, reablement, and independence.

Key Responsibilities

• Conduct comprehensive client assessments and develop care plans

• Coordinate in-home services and liaise with external providers

• Provide education, guidance, and advocacy for clients and families

• Monitor care budgets and ensure compliance with aged care standards

• Build strong, trusted relationships with clients and stakeholders

About You

• Current AHPRA Registered Nurse registration

• Experience in aged care, community nursing, or care management

• Sound knowledge of the Support at Home Program and aged care standards

• Strong communication, organisation, and time-management skills

• Current NSW Driver’s Licence with a reliable, insured vehicle

Requirements

• Valid AHPRA registration

• Pre-employment medical, police check, and required vaccinations
