Role Summary:

As the Team Leader, you will not only guide and support a team of support workers but also bring a culturally informed, participant-centered approach to service delivery. You will work closely with community partners and local employers to ensure participants are supported in culturally safe environments that empower them to pursue their goals, including meaningful employment.
This role blends team leadership, care coordination, cultural guidance, and community engagement, grounded in the principles of self-determination, cultural safety, and strengths-based practice.
Key Responsibilities:

Lead, mentor, and support a team of support workers to deliver high-quality, culturally safe care.

Oversee daily team operations, ensuring strong communication and consistent service delivery.

Work one-on-one with participants to explore strengths, goals, and employment aspirations to provide tailored strengths-based support.

Promote cultural safety, self-determination, and empowerment, collaborating with Elders and community leaders to ensure culturally informed support.

Oversee participant onboarding, plan monitoring, documentation, and care-hour utilization.

Maintain accurate records and support risk management processes.

Engage local employers to create inclusive and culturally safe employment opportunities.

Support community outreach, referrals, and partnership development to promote participant outcomes.
What You’ll Bring:

Experience in team leadership, care coordination, or NDIS service delivery.

Experience supporting people with disability and/or individuals involved in the justice system.

Strong understanding of Aboriginal cultures, communities, and cultural protocols.

Excellent communication, relationship-building, and problem-solving skills.

Ability to manage caseloads while supporting staff and engaging with community partners.

Commitment to cultural safety, empowerment, and strengths-based practice.

Relevant qualifications in community services, disability support, employment services, or related fields
