Transport: Transport Support – Physio Appointments Transport support required for Alan to attend in-clinic physiotherapy appointments on Tuesdays. Tasks: - Pick up Alan from home and transport to physiotherapy clinic at 415 Upper Heidelberg Road, Ivanhoe VIC 3079 - Physio session is 45 mins. - Wait or return to collect after appointment and transport back home - Please advice if longer service hours required. Level of assistance: One support worker Equipment in use: Foldable wheelchair Mobility / Transfers: Alan requires one or two-person assist for transfers; may require assistance getting in and out of vehicle Special instructions: - Vehicle must have sufficient space to store a foldable wheelchair -
