The people you provide care and services to are partners with you and your service as a requirement of the Aged Care Quality Standards. Good partnerships come from open and honest communication. 

So when harm or a near miss has happened, people need to know, and trust it will be sorted out in the best way for them. 

When mistakes get covered up or are not reported the people you provide care and services to and others can be at risk of serious harm.
