PHM Health began its journey back in 2016 in a town called Narrandera, NSW. Back then the focus of the organisation was small and ad-hoc community support for seniors and the disabled. Upon expanding into the Sydney Metro a year later (2017), the organisation was presented an opportunity to again provide care to people who were aged and disabled, which was now done via the NDIS. Thus, PHM Health began as an NDIS unregistered provider. Over the course of that year (2018), the organisation experienced rapid growth and decided to head down the road of becoming a registered NDIS provider.
Following this growth and demand for PHM Health, 2019 saw the organisation take a number of key senior and administration staff and relocate them to a new office at Belfield. Although the office was small, it gave the organisation a new focus which again resulted in even more growth.
