The Role:

As Team Leader, you will be responsible for overseeing activities across all areas of Oyster Care regarding Support Workers and our Participants. In addition, you will be expected to aid Support Workers and Participants whenever required, develop problem-solving strategies, and cooperate with other teams within Oyster Care to improve organizational effectiveness.

Key Responsibilities:

Participant Services

  • Ensure the relevant needs of the participants are identified and met.
  • Monitor participant files (including shift reports) and ensure all information is current, accurate and updated in a timely manner.
  • Participate in the person-centered planning process for participants and monitor outcomes, including attending case conferences.
  • Review, maintain and update participant risk assessments, reporting all hazards to management promptly and taking immediate action to mitigate risk.
  • Assume the role of key worker for designated participants, including the preparation of a three-month report on participant progress.
  • Develop and follow up NDIS plan reviews.
  • Follow up on participant goals to ensure implementation into service provision.
  • Investigate incidents and report follow up actions to the Manager.
  • Liaise with Support Coordinators and other stakeholders involved in the care of the participant.
  • Identify and liaise with other community services/departments to meet participants needs.
  • Supervise and assist participants as required, including filling vacant shifts if required.

Staff Management

  • Provide supervision, guidance, and support to designated Support Workers. This includes being present for a new employee’s ‘buddy’ shift/s.
  • Distribute WHS equipment (such as PPE bags) to designated staff.
  • Liaise regularly with designated Support Workers to ensure that they remain engaged with Oyster Care.
  • Complete and oversee annual performance development process for designated Support Workers.
  • Undertake performance management of underperforming staff as required.
  • Ensure designated Support Workers are participating in staff training.
  • Present face-to-face training with staff.
  • Ensure that all staff have appropriate training and support to achieve the agreed service delivery.
  • Monitor rosters and timesheets within designated team, including assisting with the organization of relief support.
  • Ensure designated Support Workers are compliant with requirements of their employment and that relevant checks (such as WWCC and NDIS Worker Check) are updated as required.
  • Be vigilant in monitoring individual work practices and report any concerns with respect to health and safety immediately to Oyster Care.

Operational

  • Participate in the auditing process.
  • Participate in the planning of education and training.
  • Provide a monthly report to the Manager on service issues and outcomes.
  • Present relevant reports at designated staff meetings.
  • Conduct initial investigation of complaints or incidents and assist with related documentation.
  • Liaise with scheduling regarding identified participant rostering needs.
  • Contribute to the continuous improvement process.

Competencies

  • Must always understand and adhere to relevant company policies and procedures.
  • Maintain and promote the WHS policies and systems.
  • Participate fully in WHS audits and investigations as required.
  • Maintain standards relating to Quality/Environment /Security
  • Complete all mandatory training as required.
  • Ensure the National Standards for Disability Services are always adhered to.
  • Excellent time management skills with the ability to multitask and prioritize workload.
  • Ability to build effective relationships with participants and their representatives to facilitate a person centric support structure.
  • High level interpersonal skills in dealing with managers, colleagues, clients, and key stakeholders.
  • Work as part of a team and autonomously when required.
  • Understanding of privacy and confidentiality issues
  • Ability and willingness to learn; Self-motivated.
  • Outstanding verbal and written communication
  • Intermediate IT skills
  • Ability to motivate and upskill employees, Leadership skills.
  • Knowledge of relevant legislations and authorities which impact on your area of responsibility.
  • Ability to work after hours on a rotating roster, including on-call duties.
  • Willingness to travel, including to regional NSW and interstate.
  • There may be a requirement from time to time to perform other duties as per the organizations need or request of the Manager.
