APM Communities delivers NDIS Local Area Co-ordination, Partner in the Community services in a number of service areas across WA, NT and QLD. Our team helps NDIS participants and people with disability, families and carers to identify and access the support they need to enjoy a better life from community, mainstream and funded disability services.

Local Area Coordinators

Our Local Area Coordinators assist people with disability to access the NDIS, working directly with participants of the Scheme, their carers and supporting family members. We are also available to assist people with disability who are not eligible or who chose not to access the NDIS, so they can access other community and mainstream support services.

Creating opportunities for everyone

As part of our local services, APM Communities also provides practical advice, information and support to businesses, mainstream agencies and community groups to improve services for people with disability. We aim to increase access and inclusion in the communities we work in through projects and partnerships that develop more opportunities for all.
